1. Application to the Head of the SVR for acceptance of the dissertation for consideration in a specialized scientific council (application is written in his own hand).
2. A photocopy of the first page of the passport of the applicant, certified by the signature of the scientific secretary of the specialized academic council (2 copies);
3. Identity card (form P-2DS), approved by the order of the Ministry of Statistics of Ukraine dated 26.12.95 №343, registered at the Ministry of Justice of Ukraine on 28.12.95 under №484 / 1020, with information valid at the time of submission of documents to the specialized academic council, certified by the signature of the head of staffing service and the seal at the main place of work, with a photo on which the institution's seal is affixed (2 copies);
4. Copy of the Master's diploma (specialist), certified by the signature of the scientific secretary of the specialized academic council (2 copies);
5. Certificate of passed candidate exams in the form determined by the Regulation on the preparation of scientific and pedagogical and scientific personnel, approved by the resolution of the Cabinet of Ministers of Ukraine dated 01.03.99 №309 (in 2 copies: one copy - the original, the other - a copy);
6. Excerpt from the order on enrollment in postgraduate and doctoral studies, certified in accordance with the current legislation, if the candidate for a Ph.D. degree is in graduate school;
7. Copy of the diploma on the scientific degree of the candidate of science, certified by the signature of the scientific secretary of the specialized scientific council (2 copies);
8. Conclusion on the scientific and practical value of the dissertation, issued by the organization where the dissertation was performed or to which the applicant was attached (2 copies). The conclusion is formulated as an extract from the minutes of the meeting of the department, signed by the head of the department, approved by the head of the institution and affixed with a seal. The opinion is valid for 1 year from the date of its approval until the submission of the dissertation for consideration by a specialized academic council;
9. Response of the scientific adviser (scientific adviser), certified by the seal at the main place of work of the scientific adviser (consultant).

To the documents are attached:

• a dissertation, prepared in accordance with the established requirements, in the number of copies, bound and signed by the author, necessary for transfer to the library of the institution where the specialized academic council, the specialized academic council and for sending to the opponents are created;
• dissertation abstract, designed in accordance with established requirements and signed by the author on the cover (2 copies);
• monographs, copyright certificates (copies), articles (copies), printed abstracts, reports and other materials of scientific conferences, congresses, symposia, seminars, schools (copies), listed in the abstract. Copies of scientific papers are certified by the signature of the scientific secretary;
• abstract of the candidate's dissertation protected by the applicant (for PhD candidates);
• a CD-R or CD-RW CD that contains an abstract text file. File name - aref.pdf (2 copies);
• CD-R or CD-RW CD containing a text of dissertation. The file name is dis.pdf (2 copies).
